covid vaccine

On Monday, April 12, 2021, El Dorado High School will host a COVID-19 vaccination clinic on campus in the EHS Arena. The Medical Center of South Arkansas will be administering the Pfizer COVID vaccination to EHS students who are 16 and older. All vaccines are free of charge, and insurance information is not required at this time.  

Students received forms in their English classes, as well as emailed directly to their ESD student email account. You can also find the forms HERE. In the packet, you will find information such as a vaccine consent form, a patient fact sheet, and a CDC V-Safe program sheet. Please review the forms and complete them as accurately and neatly as possible. Information will be used to input data into the students’ state vaccination records, and vaccine cards will be handed out at the time of vaccination. 

This vaccination is a two-shot series--you must have both shots to be fully vaccinated. The second shot is scheduled for Monday, May 3, 2021. 

If you would like for your child to receive the vaccine, you must complete the consent forms and return them to school no later than Friday, April 9, 2021. No late entries will be accepted. Please make sure all areas of the forms are filled in correctly.
